Hormone/Weight Loss Consultation - $225.00*

New Patient Visit-Concierge Program - $200.00*

Physical Exam/Well Child Check- Concierge Program - $200.00*

Existing Patient Visit- Concierge Program - $150.00*

 *Charges for additional in-office testing and/or procedures will be added to the self-pay visit charge as applicable.
