CONCIERGE MEMBERSHIP

  • Patients have direct cell phone access to the Provider after office hours for guidance regarding urgent concerns that arise unexpectedly. Video chat and text messaging may be used when the Provider agrees that it is appropriate.

  • Patient shall be given the Practice’s e-mail address and patient portal access through the electronic health record to which non-urgent communications can be addressed.

  • Coordination of medical records and/or peer-to-peer consultation with emergency facilities will be provided as necessary to provide continuity of patient care.

  • Provider will coordinate with Patient’s medical specialists to assure continuity of care, and if necessary, Practice staff shall assist in obtaining a referral and scheduling an appointment for specialty care.

  • Practice will assist in scheduling appointments at Patient’s radiology center of choice. Practice will supply radiology center with required prior authorization information and Provider will be available for peer-to-peer appeals when necessary.

  • Practice will assist patient’s pharmacy of choice with submission of prior authorization information to assist in processing pharmacy benefits for medications prescribed by Karibu Family Care providers.

Concierge primary care increases the availability of patients to access our personalized care and communicate with our Karibu Family Care providers in exchange for a monthly membership fee, highlighted below. Patients will still be able to use their health insurance coverage and benefits and will maintain the responsibilities for the coverage of services under their respective policies.

Monthly concierge fees will be:

Individual Adult Membership - $100/month

Individual Child (0-17 years) Membership - $50/month

Family Membership (2 parents, all children under the age of 18) - $275/month

Anthony and Amy will be limiting their patient panel to only 1500 patients total, allowing them to provide exceptional personalized experiences such as:

Exceptional Personalized Care

  • Same/next day appointment availability.

  • Longer appointment times.

  • Telehealth appointments when deemed appropriate by the provider.

  • A direct phone/text/email contact for Amy and Anthony will be available 24hrs/7 days/365 days.

  • Eliminating the need for a patient’s trip to urgent care on weekends or when traveling out of state through access to telehealth appointments with Amy and Anthony.

  • Assistance with imaging studies coordination/scheduling.

  • Assistance with specialist referral coordination/scheduling.

  • Access to weight management programs.

  • Limited in-office lab draws.
